PATIENT INFORMATION

We welcome new patients and hope you will always feel comfortable and relaxed while in our care. Children are welcome in our practice, too!

Your initial appointment will consist of an evaluation and explanation of your dental health, diagnosis and treatment options.

Please help us by bringing the following to your first consultation:

• Any X-rays (if applicable)
• A list of your current medical prescriptions.

If you have medical or dental insurance, please have your card available.

Payment Information: Our dental office is in-network with Blue Cross Blue Shield of Tennessee and Delta Dental, and will be happy to file any insurance as a courtesy. We also offer payment plans through CareCredit, a low minimum monthly payment program.
